What is MyChart?

MyChart offers patients 14 years of age or older personalized and secure online access to portions of their medical records. It enables you to securely use the internet to help manage and receive information about your health. With MyChart, you can:

  • Schedule or request medical appointments.
  • Cancel or reschedule existing appointments.
  • Have an e-Visit with an Eskenazi Health provider.
  • Review health summaries from outpatient appointments or inpatient stays.
  • View test results.
  • Request prescription renewals and refills.
  • Access trusted health information resources.
  • Communicate electronically and securely with your medical care team.

Is there a fee to use MyChart?

MyChart is a free service offered to Eskenazi Health patients.

How do I sign up?

Patients who wish to participate will be issued a MyChart activation code during their visit. This code enables you to log in and create your own username and password. If you were not issued an activation code, you can ask to sign up during your next office visit or sign up online by visiting https://mychart.eskenazihealth.edu/MyChart/signup

Who do I contact if I have more questions?

When can I see my test results in MyChart?

Most test results are available within three days. Some take a little longer and are released within 21 days.

If some of my health information on MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your provider’s office. Ask your provider to correct any inaccurate information at your next visit. Your health information is reviewed and updated in your electronic medical record each visit.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within one to three business days. Please note that MyChart should not be used for urgent situations. Please contact your provider’s office if the situation requires immediate attention or dial 911 in the event of an emergency.

Can I view a family member's health record in MyChart?

Yes you can. This is called proxy access and allows an individual to log in to his or her personal MyChart account and then connect to information regarding a family member. Proxy access is not limited to parents and/or guardians and children. Proxy access can also be granted for the following: husband/wife, partner/partner and adult child/aging parent. Ask for a proxy access form from any Eskenazi Health site where you are treated and complete it to request access to this convenient service.

Can I ask questions regarding a family member from my MyChart account?

No. If you have proxy access then you can ask a question from the proxy’s account but you should never ask a question about a family member from your own MyChart account.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each individual must establish his or her own MyChart account.

I forgot my password. What should I do?

MyChart provides two-factor authentication so you can reset your password to your email or mobile device. You do not need to call anyone to reset your password.

Can you send me a new activation code as I have lost it, let it expire or did not receive it?

You may sign up online by visiting https://mychart.eskenazihealth.edu/MyChart/signup to get an activation code or visit an Eskenazi Health location to sign-up in person.

Where can I update my personal information (e.g., home address, e-mail or change my password)?

Log into MyChart. From the top menu, select Profile then select Personal Information under Preferences & Info.

How is MyChart secure?

Eskenazi Health takes great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames and passwords. Each person controls his or her own password, and the account cannot be accessed without that password. Unlike conventional email, all MyChart messaging is done while you are securely logged on to our website or mobile app.

I was logged out of MyChart, what happened?

Eskenazi Health aims to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need to use MyChart?

You need access to a computer or mobile device connected to the Internet and an up-to-date browser (such as Internet Explorer). You can also download the MyChart app for access to your MyChart account on the go.

My activation code does not work, what should I do?

For your security, the activation code generated at a visit expires after 30 days and is no longer valid after the first time you use it. If you still have problems, please call our MyChart technical support line at 317.880.CARE (2273).

Yes. Please ask for information about Lucy and MyChartCentral. Lucy and MyChartCentral can also be accessed through MyChart online. Select the Health option from the top menu then select Lucy under Medical Tools. Once you follow the prompts to set up a MyChartCentral account you will be able to link to other MyChart accounts.
